At Health Network of Wisconsin we provide a comprehensive list of services that can be personalized to meet the needs and interests of your loved one. We can provide services in their own home or in our Adult Family Homes located in Greendale, Brookfield and New Berlin. Initially, we start by visiting with you and talking about your challenges and lifestyle needs. This initial assessment helps us customized a care plan that focuses on family schedules, the cares and needs of your loved one and provides solutions for the problems you are faced with. All of our services are developed, carried out, and monitored through our Quality Assurance Process, which is constantly monitored to reflect the changing needs of your loved one.
If the yard work and maintenance of the home is becoming too overwhelming, you can also consider our Adult Family Home. Oak Haven of Greendale is a ranch style – 4 bedroom home that provides a nurturing and loving family atmosphere for 4 seniors. This option provides peace of mind to adult children without all of the common stress and anxiety that comes along with maintaining a home. For more information, please call us for a complimentary home visit.
Health Network of Wisconsin is seeking to fill a vacancy in our Adult Family Homes located in the Historic Village of Greendale. This well-maintained sprawling ranch features 4 Bedrooms, 1 1/2 baths, a family room with a natural fireplace and a beautiful backyard with a large patio area. OAK HAVEN OF BROOKFIELD
Simple, caring companionship is one of our most frequently requested services. A Health Network of Wisconsin caregiver is a trusted companion that can help your loved one stay socially engaged at home, and also accompany them to visit friends, family, and other social events. While studies show that feelings of loneliness can sometimes lead to anxiety and depression, in-home care can provide meaningful companionship for your loved one - just knowing they have a friendly, compassionate companion who visits with them regularly.
Health Network of Wisconsin provides a "network" of health care options and resources to help you remain safe and secure in the comforts of your own home.
